SOAP formatting is a structure for organizing source notes. It should make review easier, not add details, interpretations, or unsupported clinical facts.

SOAP structure at a glance

Subjective is information reported by the patient or source. Objective is observed or measured information from the source text. Assessment contains the assessment wording already supported by the source. Plan contains follow-up, tasks, or plan text from the source. A formatter should preserve meaning and keep uncertain text visible for review.

Fictional example

A rough study note might say: “reports headache today, vitals copied here, exam note from source, follow up instructions from instructor example.” A formatted study version can separate the text into SOAP headings while leaving the content unchanged for review.
